Yeah u can port foward or set it up with Hamachi.

There are numerous things that could be causing this. You should also be more specific like whether your server uses Hamachi or not. I would recommend looking for a troubleshooting page for MC servers.

Either:

a) Look up how to Port Forward

b) Download some kind of program that connects players, like LogMeIn Hamachi or Tunngle.
