That did that to me when you leave your game it will say sometihing had an error and won't save your game

This happens sometimes, normally it happens when the game dont load the textures properly due to connection and/or disc scratch.

Someone did it about a week after the map came out stating the same thing you did "Mod/Glitch". It's just an error, get over yourself.

It's not disc scratches.

I had the same thing happen once playing 4 player but it was on less of a scale.
