Well, first things first, there are different files to mod for GTA IV, not just one.
There's common / .rpf mods, which include car speed / drift handling, flying cars, hud colors, walking styles, plus much more, basically, the main file for most of the mods.
There's also car spawns / map mods which are cars and ramps spawned everywhere on the map.
And then there's script mods, which include all the fun stuff, like godmode, teleporting, spawning cars, spawning objects, plus much much more.
