From what i remember(haven't gotten on my RGH for a long time now)

You go to XeX menu and find the folder where you have Cod4 Installed(should be the folder with a bunch of files)
Now that you know which folder it is you go to the folder where you have the Patches, Click RB or Y cant remember but it should come up with some options click Copy on the Patch.
Then go back to your CoD4 Folder and Click RB or Y and Select Paste.
If a Pop up comes up then select over right or replace.(some Patches come with modded Default.xex files do the same and copy and paste them into the folder)
Then click the Right Joystick to get to the Default files, start up the default.mp.xex and go to system link(dont change anything but the map)
Join with your retail and click the Stat or back button to open up the menu or mods(depends on what kind of patch you have)

this is for MW2, you wont need a .Dll file if your on Cod4

Put the patch in the Cod 4 Folder. Launch the default.
Simple. Just make sure that the game is updated and so is the menu to the right TU so it works.

I put the patch in the cod4 folder and it still comes up with a regular game
