If you previously completed the Call of Duty Ghosts campaign on current-gen consoles, you can do it again on next-gen and earn those achievements & gamerscore over again.

UPDATE: I would believe that this works for other games as well that are being brought to next-gen.

It's not an exploit, 360 is on a different server to the X1 that's why they unlock again, and cant speak or play with 360 users.
