If you were modded then you can just call Activision and they will reset them for you if you request it. (Only if you were modded to the level/stats)

They stopped resetting as of 2/14/13

Get into a challenge lobby and select what prestige you want. You wont be able to get reset back to level 1, non-prestige but it'll put you back to prestige 2, level 1 with nothing unlocked.
