I had a pair of XO fours for a while and had the same problems. Some times i could fix it by quitting the current game session and reload it. When that didnt work, i had to hard reset the box (hold power button the console until it shuts off) and it always worked after that.

I think it has something to do with being in a party i have a coupple of friend with this problem so we just started to use skype which fixed it for them.
