Hey guys I've been having this same problem for awhile where I constantly am disconnecting while playing games.
It sends me to the main menu and says "You have been disconnected from the Call of Duty Black Ops III Servers"
I have 60 download speed and 5 upload so speed is not the problem. This is happening on my PS4.
