I would say just go and get her something to commemorate your 4 years of being best friends. The underwear is a good idea, but it's something that a boyfriend should do, not a best friend. Just make a collage of your guys pictures together or make a necklace when maybe some things your guys have collected over the years.

Now if your wanting to take a step up and risk being her boyfriend I would still say make something that shows how much fun you guys have had in the last 4 years and then tell her how you fell (If thats the route you want to take)
