That budget is low for what you're expecting you could get a rx 480 for 233$ USD or a 470 for a bare minimum of 173ish or right up to that 200$ mark. You will not be getting that performance for that price, but you could get away with medium/high settings.
Nobody knows his specs so no one can really recommend a card.

Depends on your current specs. You don't want to bottle neck your CPU.
