the save just in front of yharnam queen boss fog gate.kill the queen and the trophy will pop up.
It will save you couple of hour through chalice dungeon to get this trophy.

modded character with inf health sure you cant die.

-load char name `Grim` lvl 544 and kill the Yharnam Pthumerian Queen and get the 'Yharnam,Pthumerian Queen' trophy
