Okay so for this one right when you start there is an option for kneel if you press X so go ahead and kneel this will give you a trophy for joining the dark moon covenant.
Then go to offer souvenir of reprisal give at least 30 which will give you your final miracle.
Now if you want another trophy you can proceed through the fog door and defeat dark sun gwyndolin for another trophy.
