Hope these help, it could honestly be something as simple as plugging your controller in and updating it via settings.
If it's still doing after watching these videos I would recommend checking and possibly contacting Xbox support. |

I personally don't know a great deal about the Xbox as I am on PC, wish I could help more, I have the linked controller troubleshoot page below encase you haven't already checked/followed it.

It is very possible maybe do some research a virus can be inputted into a controller from a pc a controller has some sort of memory for it to remember consoles the for it is possible but very unlikely still can happen tho all that rep and you failed to do a quick search |

waterboy2 wrote It is very possible maybe do some research a virus can be inputted into a controller from a pc a controller has some sort of memory for it to remember consoles the for it is possible but very unlikely still can happen tho all that rep and you failed to do a quick search I have built PC and Game Consoles there is no way the controller can give you a virus on your Xbox or PC. Only consoles that had memory card was the original Xbox. So if that's what your talking about then it's something you added to the memory chip. So you have no clue on wtf your doing and downloaded the virus and added it yourself. |
