The brain surgery is tough to avoid losing blood in since every time you hit the skull, his blood level will drop. I found that using the flat side of the axe and rubbing it around his skull will chip most pieces away with the least amount of damage. Use it until you have most of the brain exposed, and if you need to chip a little on the sides to get the brain out, use the hammer.
Any direct impact to the brain itself will cause massive blood loss and you'll have to restart. Once you have pulled the brain out, make sure to stab him with the needle before cutting the stem if there's any gradual blood loss. Then cut the stem and put in the new brain.
