Perform a double kidney transplant in an ambulance in under 2 minutes.
This double kidney transplant has to be completed 1 full minute quicker than the last time trial where you earned Don't In-Test My Patience. You will need to be exceptionally fast for this one. I would recommend grabbing the bone saw on the left side of the ambulance at the beginning and use this to cut everything including the yellow veins attached to the kidney's before letting go of the tool. You can cut them with the intestines still in the body.
Once you have cut everything and removed everything, the trick will be to placing the two kidney's inside of him. You will need to place one in its' correct location and then quickly place the other one in before the first one fly's away. Watch this video to see it in action:
