All of the ambulance timed trials are very difficult, but this is the easiest of the bunch. You actually have 30 additional seconds from the first stage brain transplant. Use the axe, or hammer to cut out the skull to reveal the brain. Just be careful that you don't hit the brain itself as that will become a quick death for Bob.
After you expose the brain, pull it out and cut the brain stem with a scalpel or scissors and then place the new one in. This video will show you the way:
