If you have this error and cannot access the recovery menu to rebuild the data base, then this means that your hard drive is most likely dead.
in order to test this you must have a dock, remove the drive from the ps4 and place it into a dock on your pc and then open a hard drive utility tool such as easeus partition master. then search for your ps4 drive in there, if it shows up then your drive is most likely corrupted and still usable, you will have to reformat this drive with your computer to any format just to clean it up and then reinstall it to the ps4 and follow on screen prompts, you can follow my video from after i replace the drive for help with this section.
OTHERWISE if your drive does not show up at all in a hard drive utility tool then it is probably dead :/ so in this tutorial i will show you how to use a newly obtained drive to replace it for the system and bring her back to life
First of all we will need a hard drive to replace the original. You can obtain one from local computer stores usually or Ebay, in my opinion these are usually expensive and unnecessary, so i have purchase a Seagate slim backup plus hard drive for a lower price and torn it apart so we could use the drive inside the case. Do not fear this is easier than you would think just watch my video showing you how to do this. these drives are usually more accessible for people who are not near any computer stores or who do not wish to purchase products online.
Be careful if purchasing external drives for use as some do not have sata connectors within and cannot be used for the Playstation, make sure to look at taredowns of the drive before purchase and make sure they are the right sizes to fit in the PS4. i believe they cannot be any thicker then 9mm

First make sure the PS4 has no cables plugged into it remaining. then you are going to want to slide the glossy cover away from the PS4 until it pops and you can remove it.
After you have taken this off you will need to unscrew the Playstation screw with the button symbols on it so we can access the Hard Drive bay.
When you have unscrewed that screw the Hard Drive should be able to slide out and you will then have to unscrew it from the Cradle
Remove it when all screws are removed and place new Hard Drive in Cradle and use removed screws to fasten it.
Slide Cradle back into bay and screw Playstation screw back in the same spot.
Make sure everything is correct and moves into place then put glossy cover back on PS4 and you are done with replacing.
Boot PS4 back up and it will ask for a firmware file of the most recent firmware it had installed or above, you will be required to locate one for installation. The latest OFW can be found HERE
Once you have this Firmware you must place it on to a FAT formatted usb like so. On the root of USB you must make a Folder called PS4 --> UPDATE --> PS4UPDAT.PUP
Place this USB in to PS4 when it asks for the firmware and then follow onscreen prompts to install it. This will now format the new Hard Drive and install the firmware, this will take a while.
Once this is complete it will ask to be set up like a new system so you may set it up as you would normally do. Then it will boot to home screen with no data.
And that is it your system is back to life and you can continue using it after reinstalling all your apps and games and what not which shouldn't be too hard if you have ps plus. hope this helped you guys who thought your systems were dead and saved some money that you would spend sending it to $ony, also if you guys tried this and bought external drives other than the ones i have listed would you please post the names of the drives you used and a picture into the comments so i could add them to the list thanks guys have a good one.
