Human skulls are items in which you will find throughout the game either in chests or just randomly on their own. There are more than 25 in a single playthrough, and none of them are missable as long as you find them all before you fight the last boss as this is the point of no return. You will find plenty of Human Skulls in the Graveyard and Catacombs/Tomb area of the game. I dont believe a guide is required for their locations. As long as you are searching everywhere, you go you wont have any issues finding 25. You can trade 15 Human Skulls with the Crippled Rhogar for a Special Weapon (he is located immediately after the fight with the Infiltrator boss). Just be sure to have either already unlocked this trophy, or simply just obtain the special weapon on your second playthrough.
