After you defeat the First Warden and exit the Monastery to the outside snow area, you will need to open the gates to leave the courtyard. Once you open the gates, there will be a man standing between the two gates in which you can talk to. He is poisoned, and the only way in which you can save his life is by cutting off his arm. Select the correct dialogue option to perform this task and the trophy will unlock.
