In the Cave of Lies, before the boss fight against the Annhilator, you will come across a large monster that is chained up. Pull the Lever next to him to start a cutscene with Yetka. She will plead with you not to kill him. Simply select the dialogue option Ok, Ill let it live to unlock the trophy. If you accidentally select the option to kill it, you will not be able to unlock this trophy on this playthrough. Just be sure to select the correct dialogue option on either of your two subsequent playthroughs.
