This trophy is very straightforward, however one of the pieces of the blade itself is a bit tricky. The blade that you will find in the Choking Grounds is accessed through the bottom of the house. That entrance is to the left in a little tunnel, right on the path towards where the Golem used to be.

You have to make your way through the tunnel, into the basement of the house, up to the top. The trick here is that on one of the floors you will be required to use the Abyssal Chain on some Demonic Growth. It isn't obvious, and therefore stumps a ton of people (myself included).

It also should be noted that it is impossible to obtain "Battle Hardened" until you get this trophy. As long as you finish the game you will not miss this trophy. It is one of the very last things you will complete in the game.
