Score a combo of 75 or more while fighting up the stairs at the "Front Gates of EEEEEEEVIL!"

Once you get to the floating stairs, you have to get a combo of 75 or more, which is kind of tricky, mostly because of the enemies being scattered. I recommend you use the sai and machine guns for this trophy. Once you get to the stairs, teleport up to the first platform, defeat the two guys there, hurry and teleport up to the next platform that holds the mutant and the rifle man. Defeat these two guys, and hurry on to the next platform located to the right from you. Defeat the rest of the enemies.
