I personally don't think he is alive. He got shot point blank with a magnum, was covered in his own blood, drenched in gasoline, and burned. I don't even think this game is going to be a prequel. If the rumors going around are true about it taking place in the future and being kind of like an apocalyptic time, I think 'Ghosts' is just what your team is known as or likes to be called. I think they just wear the same mask as Ghost did, but none are him. If they are effective enough, they could take out the enemy so quickly and undetected that they could be considered "ghosts." Sounds like it would have more stealth involved if that were the way the creators thought of it as.

I hope Ghost ain't dead, he was pretty cool. His mask is pretty dope

I mean i imagine that the new game will probably include ghost in one way or another. Maybe they have an elaborate story about how he survived being lit on fire and shot multiple times, or its a prequel of mw2.

i don't think its possible as its based in the near future so h would be like 50 lol,
