Have you tried a different remote? I have a scuff and it will not work with that controller i have to use my original xbox remote. You may need to update controller or you could go through your settings and make it where you can hear people through tv and headset. Thats about all i know it happend to me too

You can try restart your xbox by unplugging it from the back and then try, might just be a little glitch.

Check both ends and make sure they are plugged in all of the way then check your RCA cables if you use them.
