Your wires might be the issue, Try putting CPU_rst on the top & move cpu_pll_bypass around and test.

Its likely a interference issue with your wire routing, if that doesnt work you could build ...

i wouldnt have thought so some of the points are so small the slightest shake of the box could make them come loose (which is why i cover all of he points in a nice amount of hot glue)

try reflashing the coolrunner with new files then if that does not work try resoldering all wires and it that does not work replace.
