Not trying to sound big headed but I'm pretty sure I know more than you on this topic.
Physics is a great a level to take and is linked to medicine. Go and look at some entry requirements for medical schools in the UK, you will see physics, bio/human bio, chem, maths listed. You won't see them asking for Psychology (although you could obviously take it as a 4th subject) but I don't see how a level Psychology would help OP, whereas Physics would certainly help, the amount would be difficult to say as that would depend on exam board.
So to OP: I would recommend Physics but it's not easy but you should be able to cope if you're looking to apply to medical school.
but at the end of the day if you don't think you'd enjoy physics then pick a subject you will enjoy as it is your 4th subject anyway so it wouldn't be too crucial. (although physics may help you if you performed less well in another subject when applying to university next year)

I have knowledge on the subject. I have an a level in physics. And I'm a 1st year Physics undergrad, I know quite a bit about how physics is used in medicine. I came here to help the op as the advice you were giving was quite frankly, bad, don't get mad I didn't mean to cause offence.

Physics is accepted by a vast majority of the universities that the OP would be applying to whereas Psychology isn't (it's treated as a less academic subject), I was just trying to help didn't mean to cause offence.

I doubt the fourth choice will matter, after your first year you'll no doubt have the option to drop a subject, so you only continue doing chemistry, biology and maths.

True but if he messes up with one subject (which he shouldn't do if he is thinking of medicine) but if he does then he could carry on with physics and drop the subject he messed up in i.e. maths as physics is one of the entry requirements for many medical schools.
So therefore it would be preferable to take all 3 sciences and maths IMO
