Disable plugin 5

Try to update using a flash drive.

Make sure you've renamed the file to $$ystemUpdate.

I figured you've tried most this but I believe it's your plugins

Do I need to put it on the Xbox hard drive if so where? Or do I just plug in USB like I have and update then take it out

Yea unplug the usb and do one last reset to make sure it's sticking then go on mw2 and join someone or a game to make sure you don't get a fatal crash.
