Important Tip: The Insignia trophy / achievement will only unlock after you complete the Ground Zeroes story mission. Shortly after the credits are over, you will see a screen with your rewards. Thats when it should unlock.

Note: The last XOF Unit Patch (#9 in the video guide above) requires you to roll around on the ground, and although its the last here, you can actually get that at any time you want by following these instructions Hold the X button on PlayStation consoles or the A button on Xbox consoles, then aim your weapon and hold down the left stick to start rolling. This will initiate a cutscene and the last patch will fall from the sky. Be sure you are somewhere outside when doing this. The other 8 unit patches can be picked up like normal collectibles, but they are hard to see (Protip: Use your Nightvision goggles to make them glow in the dark).
