1) Start the zombies map "Greenrun" on Normal Difficulty with a partner.
2) Earn 2,000 points which you can spare.
3) Purchase the Tombstone perk at the Power House, it will be upstairs on the catwalks.
4) Play until you die.
5) When you spawn next round, go over to the place where you died.
6) There will be a gravestone marking where you died last round.
7) Pick up the gravestone to get the weapons and perks you lost.
8) The achievement will pop up.

This will not happen if everybody dies as the next round will not start and you will not re-spawn.
