1) Start the mission "Dexter Industries - Old Mill".
2) Near the beginning of the mission you will see two soldiers playing a video game.
3) If you go over to them you will hear them arguing about the use of cheat codes.
4) Kill both of the soldiers.
5) Look at the TV screen.
6) Notice they were playing a game similar to Mortal Kombat.
