Step 1) Start the first level of Campaign.
Step 2) After you talk to Cortana, walk down the hallway, and go down the stairs to the left.
Step 3) After you have turned around, and walked down the stairs. You will see a moniter. Simply press X or B(depending on your layout) And access the Chief's record.
And the achievement should pop up.

It's a really easy achievement to get, but it's also very easy to pass up if you don't have a video like this to help you out. Nice tutorial man!
