Mission: The Storm

While making your way to the door during the mission, you will see a circle looking platform, you will need to grenade jump up there, or use a ghost to push a box over to jump up. At the top of this you will see the skull, grab it as if you were picking up a weapon.

If you destroy the Wraith by the door the skull will disappear.
