Take out the Mall's biggest customer
This is part of the story, so you will get this along the way.
On Act 5, Chapter 2, you will fight a few enemies by a crashed helicopter, soon after there will be a big enemy that will jump over some railing and attack you.
You will want to run over to the helicopter, under the blades, and face the enemy, he will wind up for a swing, and as he's about to swing at you move over a little to the side and he will hit the blades and die.

I would class this achievement in more of the medium range of difficulty. Good luck to everyone who decides to go for it :)
