From the top of full-height Agency Tower, leap into the chimney stack and live to tell the tale.
To unlock this achievement you have to be up to the last mission in the story mode. As you progress through the story, the Agency Tower is being built. Grab a helicopter near the tower and fly up to the very highest point on the Agency Tower. In front of you will be a chimney stack and you have to reach the bottom to land in the water. This may take a couple tries, but as long as you land in the water (it's okay if you die), it will unlock.

I have to say that this was a really fun achievement to go for. Jumping off the highest point in the game was a lot of fun, it reminded me of the parachute achievement from Assassins Creed: Brotherhood honestly.
