A counter is performed in a fist fight by pressing to dodge when your opponent attacks you, then following up with to make your counter attack.
To dodge you have to press (A) then to do a counter attack press (B) soon after when the enemy has his arm extended out after the block you did on him.
Do ten of these in 10 minutes to get the achievement.
Best place to do it is in times Square where there are lots of people.

I managed to get this achievement pretty early on in the game through fist fighting civilians. It isn't very hard but you need to be in the right place (with a lot of people) in order to actually pull this off in the 10 minute time limit
